Purpose
The Auditee Approval and Audit Report Release Form (PCF36) is required as part of the Exemplar Global Heavy Vehicle Accreditation (HVA) Auditor Certification process. This form ensures that the HVA Skill Examiner receives the necessary authorization from the auditee company to review and validate the audit report used in the auditor’s evaluation.

How It Works
The form must be completed by:
• The HVA Auditor applicant seeking certification.
• The Auditee’s authorized representative, providing permission to release the audit report.
• The Exemplar Global Skill Examiner, confirming confidentiality and proper handling of the audit report.

Form Sections Include:
• Section 1 – Auditor Applicant Details: Applicant’s name and Exemplar Global Customer Number.
• Section 2 – Auditee Company Details: Organization name, authorized representative, address, and NHVAS accreditation number.
• Section 3 – Skill Examiner Details: Examiner’s name, contact information, and customer number.
• Section 4 – Confidentiality Agreement: Examiner’s declaration to maintain confidentiality and destroy records after 30 days.
• Section 5 – Auditee Confirmation: Authorized representative’s approval for release of the audit report to the Skill Examiner.

Submission Guidance
All sections must be completed in full. Incomplete or unsigned forms will delay certification processing. The completed document should be submitted to Exemplar Global as part of the HVA Auditor Certification application package.
