What is the biggest issue with a risk-based approach?

Perhaps not the biggest issue, but a few thoughts occurred to me when I was considering your question. I am always mindful of blind spots, the possibility of misidentifying a risk, or incorrectly evaluating the risks that have been identified. These are examples of foundational aspects of the RBA, which, if not considered, have the potential of introducing latent weaknesses into the management system.

Blind spots, misidentification of risks, and underestimating risks are real challenges in any Risk-Based Approach. We need to ensure that risks are revisited periodically rather than treated as unchanging. It’s also important to regularly review tools such as pFMEAs and SWOT analysis. The earlier we identify weaknesses, the better positioned the organization will be to respond effectively. @Adam Maxwell
