    These events are sobering, and the hard truth is that process safety incidents rarely come from a single failure. They come from small drifts that were tolerated because nothing went wrong the last time. A facility can hold a clean management system on paper and still lose the process safety discipline that actually prevents an explosion, because the system tracks lagging indicators like injury rates while the real warning signs sit in near misses, overdue inspections, deferred maintenance and quiet changes to operating limits.

    What I keep coming back to as an auditor is that the audit should follow those leading signals, not just confirm the paperwork exists. Ask what changed recently, what alarms are being bypassed, what maintenance has slipped, and whether the lessons from the last incident actually reached the people at the valve. When I train auditors I push them hard on this, because a barrier that looks fine in a document can be degraded in the field, and that gap is where these tragedies keep finding room.

    Dilawar,

    My background is in the United States as an implementor of EHS Management Systems for Anheuser-Busch and the 12 U.S. Breweries in the 1990s which used ammonia refrigeration for the cooling of the cellars.  It was during this period that the government started addressing this issue of the release of highly hazardous chemicals from both an environmental and health and safety standpoint.  The government issued regulations that came out during that period regarding those chemicals.  In 2000, I retired from Anheuser-Busch and formed my own company consulting and auditing management systems.  I also became a member of the ANAB Accreditation Council for 20 years and an Adjunct Professor in an Environmental MBA Program for four years teaching all the EMS Classes.  I was an auditor for DQS CB for 9 years in a group of 55 auditors conducting ISO 9001, ISO 14001, ISO 45001, ISO 50001, and RC14001, and was certified to all of those management systems with the exception of ISO 50001.  I also managed this group of auditors, late in my career.  I feel that the regulations, the management systems above to include RC 14001, and the 2015 Standard revisions went a long way to address this issue of highly hazardous chemical releases. 

    Since 2022 when I fully retired at 80 years old, I began writing a book on ISO Management Systems which should be published in late 2027.

    I would be happy to talk about this subject in greater depth, just let me know.

    Chris, that is a remarkable career, and the ammonia refrigeration work across those breweries sits right at the heart of what process safety was wrestling with in that era. You lived through the exact shift from reactive regulation to real management system thinking that so many sites are still catching up on today. I would genuinely enjoy that conversation, and congratulations on the book, late 2027 gives the profession something to look forward to.

    There have been some discussions over the last week on process safety management.  There appears to be frustration with how this is being handled particularly by Community Members outside the United States.  This subject reached its peak in the United States in the 1990's.  During that period, I was the head of EHS for Anheuser-Busch Inc. and the 12 U.S. Breweries.  The breweries used ammonia refrigeration to cool their cellars.  As such, they had very large quantities of ammonia, a highly hazardous material, stored on site at each brewery.  The story below describes what took place in the United States and specifically Anheuser-Busch during the 1990's to address process safety management and the EHS regulations that were initiated during that decade.

    Hanging over our heads, was the worst chemical accident in history at Bhopal, India where 3500 people were killed and 50,000 injured by a release of Methyl Isocyanate (MIC) from a Union Carbide Plant in 1984.  Some people believe that this tragedy was what led to the development of the original ISO 14001:1996.   What Bhopal did lead to was attention to the Kanawha Valley in West Virginia which was called the Chemical Capital of the World due to the concentration of chemical plants located there, including the only Union Carbide Plant in the U.S. to manufacture MIC.  A resident in the town adjacent to the valley and plants led a campaign to find out the risks to the town posed by these chemical plants.  Instead of pretending a hazard did not exist, the chemical companies and the industry decided to address the issue posed by the town.  Working with the American Chemistry Council (ACC), the Environmental Protection Agency (EPA) and the Occupational Safety and Health Administration (OSHA), the Mayor and City Council, and the local media, each plant put up a booth in the local mall.  The booth included the risks presented by each plant; the distance from the plant that the consequences of the complete failure of the highly hazardous chemical container could have on the local community; and what the chemical company was doing to prevent that release from ever happening.  The entire event was recorded in a documentary and presented on PBS.  This 1991 Documentary became a teaching tool for not only the chemical industry but other companies with highly hazardous chemicals stored in large quantities such as Anheuser-Busch and other breweries; ice cream companies; and other non-chemical companies with highly hazardous chemicals. This documentary, titled "Chemical Valley" is available for anyone interested in learning more about the event and the communications between the chemical industry and a local community.   "Chemical Valley" documents the community's struggle over: 1) Job security vs. safety concerns - the plant's role in the local economy and the fear of losing livelihoods if it closes; 2) Environmental racism - the disproportionate environmental and health and safety burdens placed on marginalized communities; 3) Citizens' right to know and act - residents demanding transparency and accountability from industry and government; 4) Racism and job blackmail - systematic inequities in environmental decision-making.

    Shortly after this event and the documentary chronicling the event in 1990, both EPA and OSHA issued new regulations governing those issues.   OSHA issued the Process Safety Management (PSM) of Highly Hazardous Chemicals Standard (29 CFR 1910.119) in 1990.  The PSM Standard mandates a comprehensive management program covering the following elements: 1) Process Safety Information (PSI): Documentation of chemical hazards, process technology, and equipment specifications; 2) Process Hazard Analysis (PHA): Systematic evaluation of potential hazards and risk mitigation strategies; 3) Operating Procedures: Clear instructions for safe operation, startup, shutdown, and emergency response; 4) Training: Employee training on process hazards, and safe practices; 5) Mechanical Integrity: Regular inspection, testing, and maintenance of critical equipment;  6) Management of Change (MOC): Procedures to manage changes in chemicals, technology, equipment, or procedures; 7) Pre-Startup Safety Review (PSSR)  Verification that new or modified processes meet safety requirements before operation; 8) Emergency Planning and Response: Procedures to respond to releases or incidents; 9) Incident Investigation: Investigation of near misses and incidents to prevent recurrence; 10) Compliance Audits: Periodic audits to ensure adherence to PSM requirements; and  11) Trade Secret Protection: Ensuring confidential information does not compromise safety.  In order to determine the risk 

    EPA issued 40 CFR Part 355 the Emergency Planning and Community Right to Know Act - Final Rule for Extremely Hazardous Substances (EHS) and Threshold Planning Quantities (TPQ's) for Solids in Solutions.  The Regulation establishes requirements for facilities to report extremely hazardous substances (EHSs) and participate in emergency planning to protect communities from chemical hazards.  The purpose and scope of 40 CFR Part 355 is to implement the Emergency Planning and Community Right-to-Know Act (EPCRA), requiring facilities to provide information necessary for state and local chemical emergency response plans and to notify authorities of chemical releases. The regulation lists Extremely Hazardous Substances (EHSs) and their Threshold Planning Quantities (TPQs) in Appendices A and B, which determine whether a facility must comply with emergency planning requirements.  

    To conform to the regulations above for all 12 breweries, Anheuser-Busch Inc. undertook a few major initiatives by my staff and Corporate Engineering.  We first performed a Process Hazard Analysis (PHA) using a HAZOP tool.  Based on the results of the HAZOP, we launched some capital projects to minimize releases to the environment and provided alert systems to notify powerhouse personnel of even a small release from a pressure relief valve. We also redirected releases from the pressure relief valves back into the ammonia storage system. We upgraded our ammonia system preventive maintenance program.  We upgraded our release notification procedures.  Other mitigation programs such as training on the refrigeration process.  mechanical integrity inspections, employee training, and management of change processes were already in place.  As part of 40 CFR Part 355, we calculated and plotted a circle around the ammonia system to show how far from the center of the circle or powerplant the release of a complete rupture of the ammonia container could travel and provide a lethal dose of the ammonia.  With all this information in a presentation, I traveled with the Senior Vice President of Communications to each plant city and conducted a presentation with the city's leadership.  For the remainder of the 1990's and prior to my retirement from A-BI, we never had a reportable release of ammonia.

    Chris, the part that stands out to me is that the shift came from putting the real consequence distances in front of the community rather than managing the message. That is still the sharpest test I use on site, because if the people running the plant cannot tell me the worst credible release and which barriers stop it, the process safety file is documentation rather than control.
